2. Fill out all information required on our Lab Slip.
3. Make a copy of our Lab Slip for your records.
4. View and print our postage paid mailing label.

5. Place and seal all samples and our Lab Slip into a United States Postal Service approved package.
6. Attach our postage paid mailing label to the outside of the package.
7. Mail the package to DK Dental Laboratory. For other alternative shipping methods, please contact us directly.
